In my previous role as a Safety Engineer, I had to investigate an accident that occurred in our manufacturing facility. The incident involved a worker who got injured while operating a machine. To discover the causes of the accident and prevent future occurrences, I took several steps. Firstly, I interviewed the injured worker to gather information about what happened and get their perspective. Then, I examined the machine involved in the incident to identify any mechanical faults or malfunctions. Additionally, I reviewed the standard operating procedures to ensure they were clear and followed by the employees. After gathering all the necessary data, I analyzed it to determine the root cause of the accident. Based on my findings, I recommended several preventive measures, such as additional training for the employees on safe machine operation, enhancing machine maintenance procedures, and installing safety guards. I also organized a safety training session for all the employees to raise awareness about the incident and emphasize the importance of following safety protocols. As a result of these actions, we were able to prevent similar accidents in the future and improve the overall safety culture in the organization.
